Please complete the registration form and the new starter forms/booklet and return together with the supporting documents:

 

• Forms to Complete:
  • Registration Form (Word Version)
  • Registration Form (PDF Version)
  • New Starter Forms Booklet (Word copy)

• Supporting Documents we need:
  • 2 x proof of home address (for parent where the child resides) eg: Utility Bill/Council Tax/Bank Statement/Tenancy Agreement but no mobile phone bills please.
  • either a passport or birth certificate for your child/ren.
